This is a questionnaire for patients who have had surgery with Dr Walter. This questionnaire is anonymous. You are not required to give us your name or address. Your answers and comments are not connected to your medical file and will not impact on how we interact with you in the future. This also means we are also not able to respond to you personally about any of the comments you make.
The information you give us may be used to help us improve the way we help patients in the future,and we may provide relevant or important feedback to the Mater Hospital.
